R.E. Polk and colleagues at Virginia Commonwealth University studied the interactions between amprenavir and common prophylactic MAC treatments.
They found that amprenavir can prevent the clearance of rifabutin, forcing almost half of the patients studied to discontinue therapy. Conversely, rifampin was found to hasten amprenavir clearance.
Concomitant administration of amprenavir and rifabutin increased rifabutin's steady state area under the curve by almost threefold, study data showed.
Rifampin, meanwhile, was shown to decrease amprenavir's area under the curve by more than 80% ("Pharmacokinetic interaction between amprenavir and rifabutin or rifampin in healthy males," Antimicrob Agents Chemother 2001 Feb;45(2):502-508.
Polk et al. also found that while amprenavir reduces the results of the erythromycin breath test by over 80%, both rifabutin and rifampin increase the results of this test by more than 150%.
"Amprenavir plus rifabutin was poorly tolerated, and five of 11 subjects discontinued therapy," Polk and colleagues concluded. "Rifampin markedly increases the metabolic clearance of amprenavir, and coadministration is contraindicated."
